I just read the full TSB report on this 2015 accident near Thompson, MB:

It makes me think I should replace the faded placards near my fuel caps, though they didn't help in this case, and always stay to watch my plane being fueled. At least I always look into my tanks and take a sample after fueling (I've never found Jet-A, but I have found a lot of water a couple of times, at big FBOs who obviously weren't managing their Avgas storage tanks as carefully as their Jet-A).
