Anyone completed a TailBeacon installation, and the paperwork??

I have my TailBeacon Ready to install, and working on the Form 337 info.... Since examples are always good, has any one completed the Form 337 on this yet??.. My A&P / IA likes the paperwork already completed...

If not, were do I find the ICAO number??.. In Hexadecimal order??.. or where to find it??

I assume the aircraft length, width, are the actual complete length and wingspan.... is this right??

As for the GPS antenna offset, it would be length, being on the tail, 23 ft, 5.25 inches, and zero fo rthe Longitudinal offset....

I think I have all the rest of the info correct.... Thanks,
