Video looks like he was spinning a T-34 and pulled out too low, crashing into the water near Jones Island.
He was the guy on Apollo 8 who took the Earthrise photo.

Edit - I just watched a higher resolution video, and he wasn't spinning.
Looks a lot like aftermath of a moose stall. Was he turning at initiation?
