Honestly some are way premature with assigning blame or creating a “mistake”.

Recall Reno 2011 when P-51 “The Galloping Ghost” lost an elevator trim tab due to a worn out twenty year old stop nut loosening and causing the shearing of an attach bolt. At two hundred feet it rolled hard and flew right into the spectators.

It could have been incapacitation or mechanical failure. It took the NTSB a few months to figure Reno out. Give them a chance with this one.
